I, the undersigned, hereby apply for enrollment in the Coaching Course for Youth. I affirm that the applicant, whether it be my child or myself, is capable of participating in our in-person course.

I understand and acknowledge that GVF and CIDHI School reserve the right to dismiss any student at any time for misconduct or actions that may reflect poorly on the institution.

The applicant further understands that participation in the coaching course for youth is a requirement for graduation and certification. By participating, the applicant freely assumes any associated risks.

I acknowledge that parental attendance is required for minors during the trial period.
